1. Parental Consent and Authorization

I/We, the undersigned, hereby certify that I/We am/are the parent(s) or legal guardian(s) of the minor participant (“the Player”), and by completing the registration, I/We agree to the following:

a) Authorization to Participate

I/We authorize the Player to participate in the One FC Houston Tryouts, organized and operated by VISION FC (“THE CLUB”), including all related athletic activities on and off the designated field premises.

2. Media Consent

a) Image Rights

I/We understand that THE CLUB may capture photographs, video footage, or audio recordings of the Player during the tryouts for promotional, educational, or informational purposes.

b) Authorization of Use

I/We grant VISION FC and its authorized representatives the unrestricted right to use the Player’s image, name, likeness, and voice, in any format and media, globally and in perpetuity, for purposes related to the promotion of One FC Houston programs. This use is authorized without expectation of compensation.

c) Ownership

All media content produced remains the exclusive property of THE CLUB.

d) Liability of Third Parties

In the event a third party misuses such content outside the intended scope of this agreement, I/We agree that such party shall bear full liability, and THE CLUB shall be held harmless.

3. Assumption of Risk and Liability Waiver

a) Participation Risks

I/We acknowledge that participation in soccer tryouts involves inherent physical risks, including but not limited to contact with other players, weather conditions, and physical exertion.

b) Waiver of Liability

I/We, on behalf of ourselves and the Player, waive, release, and hold harmless VISION FC, its staff, coaches, volunteers, and agents from any and all liability, claims, or damages arising out of the Player’s participation in the tryouts, whether due to negligence or otherwise.

4. Medical Treatment Authorization

I/We authorize representatives of VISION FC to seek appropriate medical attention for the Player in the case of an injury or emergency during the tryouts. I/We understand that we will bear full financial responsibility for any medical treatment provided.

5. Weather and Safety Policy

In the event of inclement weather, all participants will be directed to designated safe areas. THE CLUB reserves the right to delay, reschedule, or cancel sessions due to safety concerns without notice. No refunds or reimbursements are applicable, as the tryouts are provided free of charge.

6. Data Privacy

a) Data Collection

By submitting the registration form, I/We acknowledge that our personal data and the data of the Player will be collected and processed by VISION FC for the purpose of managing tryout participation.

b) Use and Storage

This data will be used to communicate tryout-related information, track attendance, and inform us of relevant program updates. Data will be securely stored and not shared with third parties, except authorized service providers working on behalf of VISION FC.

c) Rights

I/We may request access, correction, or deletion of our data at any time by contacting

7. Final Declaration

I/We confirm that all information provided is accurate and that we have read, understood, and voluntarily agree to these Terms and Conditions, without reservation.
